Ring Dignitas. Note down the exact phrases they wish for in such an in-depth report. Contact a lawyer and request that he asks for a report from the psychiatrist concerning the comments you have noted. Say that you need this for some venture you are planning - i.e. getting a job - you will have to be inventive, I actually needed to use the lawyer during a house sale so the lawyer needed to have the information that I was 'of sound mind', 'able to make decisions for the future' etc. Your psychiatrist may well be on the ball about your plans but will know that in this way he is not liable and he may help you. However you do need to note down closely from Dignitas the points which they feel are necessary for this report.
